Athletic Team Tryout Instructions

For expediency and efficiency, please use your student Chromebook to complete and sign the Athletic Clearance Packet forms. It will automatically open in the KAMI application and allows for digital signatures.

• Item 1 of the forms below must be emailed to the Health Office at healthphysicals@ghctk12.com. 
• Item 2 of the forms below must be emailed to eligibility@ghctk12.com. 
• Items 3-5 of the forms below are informational only and do not require a signature.
